    Nice give-away! A shame that I can’t submit the form. I get a “Please fill in a valid value for all required fields”, even though I’ve filled out what I need to (I live in Denmark). Keep it up!

    Doesn’t work outside the US. You have to fill in your state. Moreover, if I fill in everything, I’m still getting errors. Good luck for my U.S. colleagues though :)

    I don’t think the form is working properly because I cant submit it. I’m from Canada and it doesn’t provide provinces so how do I submit the correct address. And even if I do pick a state, it still won’t submit the form???

    What the others said. Asking for the state when the contest is supposed to be international is kind of silly.
    Hope to get that fixed so I can apply for this awesome stuff :)

    Sorry about that guys. We’re limited in what we can do with Formstack. I’ve made some adjustments that will allow you to enter an international address. Hope this helps.

    Form is working properly as far as a US address. U-Printing is a great place to go for prints. Have used them many times.
